To open a case with The Institute for Christian Conciliation serving as case administrator, please complete the following forms (all forms are PDF files):

• Information for Individual or Organizational Representative (If any party is a corporation or organization, please also complete a Information Form for Corporation/Organization form for that party.)
• Interview Agreement
• Fees and Costs for Christian Conciliation

Please submit these required case forms, along with a check for the $500 non-refundable filing fee to:

Once the Case Administrator has received completed forms from all parties, a Mediation, Mediation / Arbitration, or Arbitration Agreement will be sent to parties. When the parties return the signed Agreement to the Case Administrator, a $1500 case administration fee is due. Once the signed agreement and administrative fees have been received, the administrator will nominate potential mediators/arbitrators and provide information for the next steps in the case.
